Any person wishing to request access to records held by Quillium Group must submit a written request using the prescribed Form 02: Request for Access to Record [Regulation 7], issued by the Information Regulator of South Africa. This form is available for download below and directly from the Information Regulator at inforegulator.org.za/paia-forms.
In terms of PAIA, Quillium Group is required to make this form available and our staff must assist you to complete it if you require assistance. Electronic submissions are accepted please email your completed form to info@quillium.co.za.
Your request must include: your full name, address and contact details; a description of the record requested; the format in which you wish to receive it; proof of identity; and if requesting on behalf of another person a letter of authorisation and a copy of that person's identity document. A request fee and access fee may be charged in accordance with PAIA Regulations. We will advise you of any applicable fees before processing your request.
Quillium Group may refuse access to records on grounds set out in PAIA including where disclosure would unreasonably disclose third-party personal information, prejudice our commercial interests, or be contrary to applicable law.
